This is what the medication is for. It changes appetite and fullness at the level of the gut and brain, which is the part willpower never reached. In the trial that tested it, people on semaglutide plus counseling lost 16% of their weight over 68 weeks. People on the same counseling without the medication lost under 6%.

What happens in the program?

Week 0, the consult
Thirty minutes in person at Reese Blvd. Health history, medications, family history, what you have tried. Blood pressure, weight and body composition. If labs are recommended, they are drawn here or ordered the same day.

Week 1, the first injection
The prescriber writes the order. Your first injection is given here and you are shown how to do it yourself.

Weeks 2 to 20, the ramp
The dose goes up in steps, roughly every four weeks, and only if you are tolerating it. You check in every week, sooner if you feel rough. Nausea is common in the first days after a dose increase and it settles. Constipation is common throughout and it is manageable. If a step is too much, we hold or go back one.

Month 4 onward, the working dose
This is where the weight comes off in the trials. Monthly weigh-in, and a review of protein, training and sleep. Labs are repeated if the prescriber wants them.

Month 12 and beyond

Under North Carolina rules, here's who's involved
GLP-1 medications are prescription drugs. In North Carolina a nurse practitioner prescribes under a collaborative practice agreement with a supervising physician, and the North Carolina Medical Board requires an appropriate evaluation before treatment, whether the visit is in person or by video. Which medication you get, and from which pharmacy, is covered plainly at your consult before anything is prescribed.
Does medical weight loss work? What the trials measured.
Yes, and the size of the effect is why these medications changed the field. Every figure below names its trial and its timeline.
Semaglutide
In the STEP 1 trial of 1,961 adults published in the New England Journal of Medicine, people on weekly semaglutide lost an average of 14.9% of their body weight over 68 weeks, against 2.4% on placebo. Half of them lost 15% or more. Waist size dropped by more than five inches on average and blood pressure came down.
Medication plus support beats support alone
STEP 3 in JAMA gave everyone the same intensive counseling, thirty sessions and an eight-week low-calorie diet. The group that also took semaglutide lost 16%. The group on counseling alone lost 5.7%. That gap is the medication.
Tirzepatide
In SURMOUNT-1, 2,539 adults lost 15% to 21% of their weight over 72 weeks depending on dose, against 3% on placebo. Nearly two thirds on the top dose lost 20% or more. In the SURMOUNT-5 head-to-head, tirzepatide beat semaglutide 20.2% to 13.7%, with fewer people stopping for stomach side effects.
Two years on, and your muscle
STEP 5 in Nature Medicine followed people for 104 weeks: weight loss held at 15.2%, and blood pressure and waist improvements held with it. A body-composition substudy measured what the weight was made of: about three quarters fat, one quarter lean mass, the same ratio people on placebo showed. The medication does not take muscle preferentially. Protein and resistance training protect it, the same as in any weight loss.
Safety updates. The FDA removed the suicidality warning from these medications in January 2026 after reviewing 91 trials. The warnings that remain, covering thyroid C-cell tumors in rodents, pancreatitis and gallbladder disease, are the reason the consult asks about family history and the reason we check labs.
All of the above are trial averages in people who also received diet and activity counseling. Individual results vary.
Who is medical weight loss for?
People who have tried everything else. Adults with a BMI of 30 or more, or 27 or more with a weight-related condition such as high blood pressure, prediabetes or sleep apnea. That is the same eligibility the trials used and the same threshold the Medicare GLP-1 program uses.
People who work from home and eat at their desk. Around one in four Huntersville workers works from home. The medication turns down the constant background appetite that a kitchen twenty feet away makes worse. Weekly check-ins fit between calls.

Who should not take a GLP-1?
Tell us at the consult if any of these apply. Some are a no, some are a conversation.
- A personal or family history of medullary thyroid cancer or MEN2
- A history of pancreatitis
- Gallbladder disease that has not been treated
- Pregnancy, planning a pregnancy in the next few months, or breastfeeding
- Type 1 diabetes, or type 2 diabetes on insulin without your endocrinologist's involvement
- A history of an eating disorder
- Severe stomach or bowel conditions, including gastroparesis
- Any planned surgery under anesthesia in the coming weeks (the medication slows stomach emptying and the anesthetist needs to know)

Will I lose muscle on a GLP-1?
Some lean mass comes off with any weight loss, roughly a quarter of the total in the trial that measured it, and that ratio was the same on placebo. Protein, resistance training and recovery protect it.
What happens when I stop taking semaglutide or tirzepatide?
Weight tends to come back, as with every method. In the semaglutide trial extension, people had regained about two thirds a year after stopping. A 2026 Lancet trial found that stepping down to the lowest tirzepatide dose kept most of the loss. We plan the exit with you.
Does insurance cover medical weight loss at Hydralive?
No, this is a self-pay program. HSA and FSA cards usually work. Hospital-based programs in the area bill insurance, and we will say so at the consult if that is the better route for you.
Can I do IV therapy or cryotherapy during the weight loss program?
