What happens in the program?

  1. Week 0, the consult

    Thirty minutes in person. Symptoms, history, training, sleep, medications, fertility plans, prostate history. Blood pressure, weight. First morning blood draw.

  2. Week 1, the second draw

    Another morning. Two readings below 300 ng/dL plus symptoms is the diagnosis the Endocrine Society and the American Urological Association use. Blood count and PSA come back with it.

  3. Week 2, the plan

    Results reviewed in person. Injections or cream, dose set, first injection given here and you are taught to do it yourself if you want to.

  4. Weeks 3 to 12, the settling

    Desire tends to move first, at three to six weeks. Mood follows. Energy and body composition take three to six months. Follow-up labs at six to twelve weeks check testosterone, red blood cell count and blood pressure, and the dose moves if they say so.

  5. Month 3 onward

    Visits every four to six weeks at first, then stretching out. Labs every six to twelve months for as long as you are on it: testosterone, blood count, PSA at the age-appropriate interval, blood pressure at every visit.

Under North Carolina rules, here's who's involved

Testosterone is a Schedule III controlled substance. In North Carolina it is prescribed by a physician, or by a nurse practitioner or physician assistant under a collaborative practice agreement with a supervising physician who holds the matching DEA registration, and every prescription is logged in the state's controlled substance reporting system. The first consult is in person, which is the standard we hold for a controlled medication.

Does TRT work, and is it safe?

Yes, in men whose testosterone is low on testing and who have symptoms. This section cites the two largest trials ever run.

Sexual function

The Testosterone Trials, published in the New England Journal of Medicine, followed 790 men over 65 with low testosterone for a year. Sexual desire and activity improved significantly against placebo. The TRAVERSE sexual function study of 1,161 men aged 45 to 80 found the same: more sexual activity and higher desire at six and twelve months, sustained at two years.

Mood and blood

Mood scores improved modestly in the Testosterone Trials and in the TRAVERSE depression study. Modest is the right word, and it is consistent. And in TRAVERSE, testosterone corrected anemia in 41% of men against 27.5% on placebo at six months.

Body composition and metabolism

A meta-analysis of 16 randomized trials covering 1,373 men found testosterone therapy lowered HbA1c, improved insulin resistance and lowered total cholesterol, alongside the fat-down, lean-mass-up pattern the trials consistently show. HDL cholesterol fell slightly, which is one reason lipids are on the monitoring list.

Timelines

A review in the European Journal of Endocrinology pooled the trial data on when effects appear: desire at three weeks, mood at three to six weeks, fat mass and lean mass at three months and continuing to a year, strength at three to five months.

February 2025: the labels changed

The FDA updated the label on every testosterone product to add the TRAVERSE results, remove the boxed warning language about heart risk, and add a warning about increases in blood pressure. Blood pressure is checked at every visit here for that reason.

What testosterone therapy is not for: treating tiredness in men whose levels are normal, preventing heart disease or diabetes, or building bone in older men. The trials looked at each of those and the answer was no or worse. It is for men with confirmed low testosterone and symptoms, and in those men it works.

Who is TRT for?

Men in their forties and fifties who are losing ground. Testosterone falls about 1% a year from the late thirties, and roughly one man in eight between 40 and 70 develops symptomatic low levels according to the Massachusetts Male Aging Study. Huntersville's median age is 40. This page is for the half above it.

Men who train. CrossFit Huntersville, the free-weight room at HFFA, the guys who ride out to Davidson on Saturday mornings. If the work is going in and nothing is coming out, labs. If the labs agree, therapy plus cryo for the recovery half.

Who should not take testosterone?

Tell us at the consult if any of these apply. Some are a no, some are a conversation.

  • A history of prostate cancer or breast cancer
  • A PSA above 4, or a prostate nodule that has not been evaluated
  • A high red blood cell count
  • Untreated severe sleep apnea
  • A heart attack or stroke in the last six months, or uncontrolled heart failure
  • Severe urinary symptoms from an enlarged prostate
  • Plans to father a child in the next year or two (testosterone suppresses sperm; alternatives exist)
  • A history of blood clots

We check blood pressure at every visit and red blood cell count at six to twelve weeks and then every six to twelve months. If a number moves, the dose moves.

How much does TRT cost in Huntersville?

What are the symptoms of low testosterone?

What testosterone level is considered low?

How fast does TRT work?

Injections or cream?

Injections are once a week, here or at home, and give a peak and a trough. Cream is every morning and gives steady levels, with the caveat that it can transfer by skin contact. The prescriber will recommend one based on your schedule and your labs.

Is TRT safe for my heart?

The largest trial ever run, TRAVERSE, followed 5,246 men with low testosterone and existing heart risk for nearly two years and found heart attacks, strokes and cardiovascular deaths were no more common on testosterone than placebo. It found slightly more atrial fibrillation and lung clots, and the FDA added a blood pressure warning in 2025. We check blood pressure at every visit and red blood cell count on schedule.

Will TRT make me infertile?

It suppresses sperm production while you are on it, and recovery after stopping is usual but not guaranteed. If you want children in the next year or two, tell us at the consult and ask about alternatives that raise testosterone without shutting down sperm.

Do I have to stay on TRT forever?

As long as it is helping and your labs are fine. Some men stay on for years. Some stop, and levels return to where they were. It is reviewed every six to twelve months.

Will I get bigger on TRT?

Lean mass increases and fat mass falls over three to twelve months in men with low levels, in meta-analyses. It is a shift of a few pounds each way rather than a transformation, and it depends on training.

Do I need to fast for testosterone labs?

Yes, and come in the morning. Testosterone peaks early and the guideline standard is a fasting draw before 10 or 11am.

Does insurance cover TRT at Hydralive?

No, this is a self-pay program. HSA and FSA cards usually work. If you want to use insurance, a urology practice in town bills it, and we will say so at the consult if that is the better route for you.
