Semaglutide and tirzepatide are both once-weekly injections that reduce appetite and slow digestion, and both produce more weight loss than any medication that came before them. Tirzepatide produced more weight loss than semaglutide in the one trial that compared them directly, 20.2% versus 13.7% of body weight over 72 weeks. Semaglutide has the longer track record and the only approval for reducing heart attack and stroke risk. Both need a prescriber, both take four to five months to reach full dose, and both cost roughly $300 to $450 a month at 2026 cash prices.

How semaglutide and tirzepatide work

Both drugs imitate hormones your gut releases after you eat.

Semaglutide copies one of them, GLP-1. It slows how fast your stomach empties, tells your brain you are full, and helps your pancreas release insulin when blood sugar rises.

Tirzepatide copies two, GLP-1 and GIP. The dual action is the reason it was expected to produce more weight loss, and the reason it did.

Both are injected once a week under the skin of the stomach, thigh or upper arm. Both start at a low dose and step up every four weeks, because that is how the stomach side effects stay manageable.

Which causes more weight loss, semaglutide or tirzepatide?

Tirzepatide, by a clear margin, in the only trial that put them side by side.

SURMOUNT-5 randomized 751 adults with obesity and no diabetes to either drug at its maximum tolerated dose for 72 weeks. The results were published in the New England Journal of Medicine in 2025.

Three caveats that the headline usually skips.

The trial was open-label. Everyone knew which drug they were on.

Semaglutide was capped at 2.4 mg. The 7.2 mg Wegovy HD dose did not exist yet. In its own trial, 7.2 mg produced 20.7% loss at 72 weeks, which closes most of the gap, with more nerve-tingling side effects (22% versus 6%). Nobody has yet run the head-to-head at that dose.

Men lost less than women on both drugs, by about six percentage points.

The pivotal trials tell the same story from further apart. Semaglutide 2.4 mg produced 14.9% loss at 68 weeks against 2.4% for placebo. Tirzepatide 15 mg produced 20.9% at 72 weeks against 3.1%.

One number to keep in your head: these are 16-month results. Any page telling you people lose 15% in three months is quoting a trial that does not exist.

Side effects: semaglutide vs tirzepatide

The side effect profiles are nearly identical, because the mechanism is nearly identical. Stomach first, everything else rarely.

The common ones, both drugs: nausea, diarrhea, vomiting, constipation, stomach pain, indigestion, fatigue, headache. They cluster in the weeks after each dose increase and settle for most people.

In the head-to-head, 21.3% of the semaglutide group vomited at some point versus 15.0% on tirzepatide, and 5.6% versus 2.7% stopped the drug because of stomach effects. Tirzepatide caused more injection-site reactions (8.6% versus 0.3%). Serious adverse events were 3.5% and 4.8%, in that order, which is to say similar.

The same boxed warning on both. Both caused thyroid C-cell tumors in rodents. Whether that applies to humans is unknown. Neither is prescribed to anyone with a personal or family history of medullary thyroid cancer or MEN 2.

The same list of rarer risks on both labels: pancreatitis, gallbladder disease, low blood sugar when combined with insulin or sulfonylureas, kidney injury from dehydration, severe stomach slowing, and the risk of aspiration under anesthesia, which is why surgeons ask you to pause the drug before an operation. Two things that changed recently.

In January 2026 the FDA asked manufacturers to remove the suicidal-thoughts warning from both drugs after reviewing 91 trials and a 2.2 million patient database and finding no increased risk. You should still tell your prescriber about any mood change.

In June 2025 the European regulator concluded that a rare eye condition called NAION is a very rare side effect of semaglutide, affecting up to 1 in 10,000 users. The US label does not carry it as of August 2026. The finding was specific to semaglutide. Sudden vision change on either drug means a same-day call to your doctor.

Red flags on either drug

Severe or persistent stomach pain, especially radiating to the back; yellowing skin or eyes; a lump or hoarseness in the neck; sudden vision loss; signs of an allergic reaction. All of those are a call, not a wait-and-see.

Dosing schedules

Both drugs step up slowly, and the slow ramp is how people get through the stomach phase.

Maintenance for tirzepatide can be 5, 10 or 15 mg. Not everyone needs the top dose, and a 2026 trial found that stepping down to 5 mg after a year held most of the weight loss.

The Wegovy pill, approved in December 2025, steps from 1.5 mg to 25 mg daily over three months and produced 13.6% loss at 64 weeks. There is no tirzepatide pill.

What each drug is approved for

This is where the choice stops being about the scale.

Semaglutide only

  • Reducing heart attack, stroke and cardiovascular death in adults with obesity and existing heart disease. In the SELECT trial of 17,604 people, major cardiac events fell 20%.
  • MASH, the liver disease formerly called NASH, with moderate to advanced scarring.
  • Kidney disease in type 2 diabetes.
  • Ages 12 and up.

Tirzepatide only

  • Moderate to severe obstructive sleep apnea in adults with obesity, approved December 2024.

Both: type 2 diabetes under the Ozempic and Mounjaro names. In that head-to-head, tirzepatide beat semaglutide 1 mg on blood sugar and weight, with the caveat that 1 mg is below the weight-loss dose.

Tirzepatide's heart-outcome trial in people with obesity is still running. It has a completed trial in diabetics that showed it was as good as an older GLP-1 drug, and no placebo-controlled heart data yet. If you have had a heart attack or a stroke, that difference matters more than seven percentage points on the scale.

On compounded versions. During the 2022 to 2024 shortages, pharmacies could legally compound copies. The FDA declared the tirzepatide shortage over in December 2024 and semaglutide in February 2025. Since then, a compounded copy is only permitted when a prescriber documents a clinically significant reason a patient needs a different dose or formulation. In April 2026 the FDA proposed permanently barring bulk compounding of both drugs, citing hundreds of adverse event reports for compounded products. A $199 a month "semaglutide" from a website with no prescriber conversation is a product you should ask hard questions about.

What happens when you stop

Nearly everyone regains a large share of the weight within a year, on either drug.

In the semaglutide trial extension, people who stopped after 68 weeks regained two-thirds of what they had lost within a year, and their blood pressure and cholesterol improvements went with it. In the tirzepatide withdrawal trial, people switched to placebo after 36 weeks regained 14% of body weight over the following year while those who stayed on lost a further 5.5%.

That is why every honest program frames these as long-term treatment for a chronic condition, in the same way blood pressure medication is. It is also why the maintenance-dose data matters. Stepping down is possible. Stopping cold mostly is not.

On muscle. Roughly a quarter to two-fifths of weight lost on these drugs is lean mass, depending on the trial and how it was measured. That is similar to what happens with any rapid weight loss. Clinicians commonly recommend a protein target and resistance training two or three times a week while on either drug. Nobody has yet run a trial showing one drug spares muscle better than the other, and any page claiming that is ahead of the evidence.

How to choose between semaglutide and tirzepatide

A prescriber makes this call with you, and the Obesity Medicine Association's guidance comes down to five questions.

How much weight do you need to lose? If the goal is 20% or more, tirzepatide reached it in about half of people and semaglutide in about a quarter.

Do you have heart disease? Semaglutide is the one with proven heart protection and the FDA indication for it.

Do you have sleep apnea? Tirzepatide is the one approved for it.

Have you tried a GLP-1 before and stopped for stomach reasons? Fewer people quit tirzepatide for that reason in the head-to-head. Switching between the two needs no washout period.

What can you afford, and what does your coverage say? The cash prices are within $100 of each other. Insurance coverage is not, and it changes by employer and by year. Medicare's $50 program covers both.

Neither drug is prescribed in pregnancy, to anyone with a history of medullary thyroid cancer or MEN 2, or to anyone with severe gastroparesis.

In the only head-to-head trial, yes: 20.2% versus 13.7% average loss at 72 weeks, and twice as many people reached 25% loss. The newer 7.2 mg semaglutide dose narrows the gap but has not been tested head-to-head.

Which has fewer side effects, semaglutide or tirzepatide?
